We are proud to announce the opening of our new Transitional Care Unit (TCU) with a special focus on Pulmonary/Respiratory at our premier facility, Windsor Atrium in Harlingen Texas. It is a 40 bed designated unit with staff that is dedicated and educated to work with short term acute patients.

 

Innovative approach to care unlike anything else that is currently being done in the Rio Grande Valley area.

 

We are seeking dedicated, experienced REGISTERED NURSES for the TCU. 

Responsibilities

Conduct rounds on assigned residents at beginning of shift, at least every two
hours and upon completion of shift.
Conduct rounds with physician and record visits in clinical records.
Perform narcotic reconciliation with incoming and outgoing shifts.
Review 24-Hour Report with incoming and outgoing Charge Nurse.
Provides and oversees nursing care to assigned residents.
Identify significant changes in the condition of residents and takes necessary action per facility policies and procedures.
Supervise aides in performing duties by verifying their assignments have been
completed.
Assist in implementing acute care plans as resident’s condition changes

Assist with admissions, QA, etc.

Evening admissions

Hot rack follow up

Critical patients follow up

Resident count after midnight for correct census

Audit MARs, SMARs, TARs, ADL sheets for omissions
Provide list to managers for their morning start up
Review of new telephone orders for correct components, transcription, follow up, acute
careplans, etc.

Responsible for physician calls, with the intent of better communication with physicians thereby reducing readmissions

 

Qualifications

Must possess a valid TX RN license

Experience working in a skilled nursing facility is highly preferred
